Lp(a)-lowering drugs cut the marker up to ~99% — but no outcome trial has yet read out

A June 2026 network meta-analysis (51 RCTs, 17,810 patients; Zayed et al., Diabetes Obes Metab; DOI 10.1111/dom.70944, PMID 42244216) found RNA-targeted agents lower lipoprotein(a) by up to ~99% (olpasiran 225mg Q12W), far beyond PCSK9 drugs. Yet no Lp(a) drug has proven it prevents cardiovascular events: the first outcomes trial, pelacarsen HORIZON (NCT04023552), only reached primary completion 30 Jun 2026; olpasiran's OCEAN(a) (NCT05581303) runs to 2028. Potent surrogate, unproven outcome.

Elevated lipoprotein(a) [Lp(a)] is a lifelong, genetically-set, LDL-independent cause of atherosclerotic disease, aortic stenosis and stroke, affecting roughly one in five people — and until now there has been no approved therapy that targets it. A new generation of RNA drugs changes the lowering picture decisively. According to the June 2026 network meta-analysis in Diabetes, Obesity & Metabolism (Zayed et al., DOI 10.1111/dom.70944, PMID 42244216), olpasiran 225 mg Q12W produced the largest placebo-adjusted Lp(a) reduction (99 percentage points), with pelacarsen, lepodisiran, zerlasiran and the oral agent muvalaplin also producing large reductions — dwarfing PCSK9 inhibitors (30%). A June 2026 review (Anchouche/Thanassoulis, Curr Atheroscler Rep; DOI 10.1007/s11883-026-01419-x, PMID 42234287) confirms the outcome trials are "expected to read out in 2026" but were, at press, still ongoing.

Why it matters (money + metacrisis lever): cardiovascular disease remains the world's leading killer, and Lp(a) is a large, currently-untreated slice of residual risk sitting on top of statins. If lowering Lp(a) prevents events, it opens a multi-billion-dollar preventive-cardiology category and a universal one-time-screening paradigm. If it does not, it becomes the textbook case of a powerful surrogate that failed to move hard outcomes — which is why HKG must hold the line between the two.

Conflict-of-interest note: the individual agent trials are sponsored by their manufacturers (Amgen/olpasiran; Novartis-Ionis/pelacarsen; Lilly/ lepodisiran). The anchor here is the independent meta-analysis, which itself warns of high heterogeneity (I2 = 90.9%) and funnel-plot asymmetry, and states findings "should be interpreted cautiously pending cardiovascular outcome trials." Treat manufacturer efficacy press separately from the pooled evidence.

What would prove it wrong: the pelacarsen HORIZON or olpasiran OCEAN(a) outcome trial reports that large Lp(a) reduction does NOT reduce major adverse cardiovascular events — decoupling the biomarker from clinical benefit and demoting Lp(a) from "modifiable causal risk factor" to "correlate."

Live-status check (as of 2026-07-07): pelacarsen HORIZON (NCT04023552, Novartis, 8,323 patients) — ACTIVE_NOT_RECRUITING, primary completion 30 Jun 2026, results not yet public (Novartis guided H1→H2 2026; timing slipped). Olpasiran OCEAN(a)-Outcomes (NCT05581303, Amgen, 7,297 patients) — ACTIVE_NOT_RECRUITING, primary completion Mar 2028. Amgen also started a new plaque-imaging Phase 3 (NCT07293260, 23 Mar 2026) and a primary-prevention outcomes trial (NCT07136012). No Lp(a) drug is FDA-approved as of this date.

Update note (18 September 2026, appended per L64 — the card above is unchanged). The falsifier stated above has partly fired. Novartis announced on 4 September 2026 that pelacarsen's Lp(a)HORIZON trial "did not meet its primary endpoint" of reducing cardiovascular events despite achieving "lower lipoprotein (a) levels" — carded separately at novartis-pelacarsen-lpahorizon-primary-endpoint-miss, which quotes the release in full and states its own falsifier. This closes the pelacarsen half of the falsifier's disjunction ("the pelacarsen HORIZON or olpasiran OCEAN(a) outcome trial reports...") but not the whole of it: olpasiran's own OCEAN(a)-Outcomes trial has a stated primary completion of March 2028 and has not read out, so this card's title claim — "Lp(a)-lowering drugs cut the marker up to ~99% — but no outcome trial has yet read out" — is now stale specifically as to pelacarsen (an outcome trial has read out, and missed) while remaining accurate as to olpasiran. Do not read this update as demoting Lp(a) generally; the new card is explicit that Novartis's own release frames the result as advancing scientific understanding rather than as a causal refutation, and that no regulator or publication has stated the stronger claim.

So we should

Card Lp(a) lowering as VERIFIED at the biomarker level and explicitly UNPROVEN at the outcome level — the exact verified-vs-hyped seam HKG/MKG polices. Watch HORIZON's 2026 readout; it will convert (or break) the whole causal-biomarker thesis.
